Please answer the following questions:
I authorize a full-time Y-Serve coordinator to conduct a grade, student status, honor code, and reference check to verify my qualifications.
I give my permission to include my phone number on the Service Council's webpage.
On Y-Serve's website there is contact information - the program's email address as well as the list of the leaders' names and some of their phone numbers. The public has access to the information, but the phone numbers are generally used by volunteers who text the leaders to get more information about how to get involved. Having your phone number on the webpage is not a requirement to be a Service Council member.
I am aware of the requirements of the position for which I am applying. If selected, I commit to fulfilling the responsibilities, including attending all events.
